Why it still works behind a cache
Two layers, decided in two places
Most popup plugins ask the server who the visitor is, which is the one question a cached page cannot answer. This one splits the decision in half.
Page rules run in PHP and get cached along with the page. Visitor rules run in the browser, so they are still correct on a cache hit.
